see p.23 of the current Lortone Catalog, and p.43 of the current Diamond Pacific catalog. Most distributors will stock or supply these.

I have the one show in this pic, purchases off ebay, is IMHO the best style, but do not know where it is currently available:

there is also a type that looks like a kind of gear puller that does not require a flat face to hold. I could not find a pic/source this morning, but have seen it recently.
